At Continuum of Care Rehabilitation (CCR), we accept a wide range of insurance plans to ensure that our services are accessible to as many patients as possible. Below is a list of insurance companies we are in-network with.

In-network vs. Out-of-Network

Being “in-network” means that our practice has an agreement with your insurance provider to offer services at a contracted rate. If we are “out-of-network” with your insurance provider, we can still provide care, but you may be responsible for a higher portion of the cost.

Self-Pay Option

If you prefer not to use insurance or are not covered by your plan, we offer self-pay options. This allows you to pay directly for services at a competitive rate, without the need to navigate insurance benefits.
